EvaluateWe do a lot of listening to understand the current situation. The core question is how can we help you? (or how can we help your Mom and/or Dad, etc.?). After listening to your assessment, we will ask specific questions about all aspects of activities of daily living to get the best overall picture we can get over the phone. If we feel we can provide help we recommend an in-home personal assessment.
AssessmentA registered nurse conducts an in-depth assessment for each client to determine what services are needed, and to identify risks in the home. The client’s daily routines, medical history, health status, memory issues, safety concerns and emotional well-being are all considered during this process. A customized Plan of Care is developed from this information.
Plan of CareWith the information from the assessment and input from the client and loved ones, a plan of care is developed. The plan outlines tasks and a schedule for services the client will receive. Our caregiver’s first visit will begin with a review of this plan with the client, family, agency supervisor and the caregiver.
Freedom Care provides service in private homes, assisted living communities, hospitals, skilled nursing facilities, and rehabilitation centers. Our personalized and affordable services are available 7 days a week and can range from a few hours a day to 24/7 live-in care. Our highly qualified and trained caregivers are ready to help you and your loved ones with a variety of daily activities, such as:

- Medication pick-up and reminders
Medication Management service is focused on helping seniors take their medications safely and on time.
Services provided by a Registered Nurse include:
- Review all current medications with the client and create an easy to read list.
- Send the medication list to primary physician for review.
- Organize 2 weeks of medications using two 7-day Pill Packs
- Provide list of contents in each area of the Pill Pack
- Contact pharmacy and/or physician for refills assuring medications are on hand
